Breast Augmentation Procedure: 4 Key Decisions

There are a number of things to consider prior to undergoing a breast augmentation procedure. In addition to addressing your personal preferences and goals, you and Dr. Pirani must also decide upon 4 key choices: 1) implant type, 2) implant size, 3) incision placement, and 4) the plane of insertion. A breast augmentation procedure usually takes 1 to 1.5 hours, and is performed under general anesthesia. The steps of the procedure are as follows:

  1. The surgeon makes the incision in the chosen location, such as under the breast fold, around the areola, in the armpit, or in the navel.
  2. The surgeon creates a pocket for the implant, either under the breast tissue or under the chest muscle, depending on the implant placement option.
  3. The surgeon inserts and positions the implant, either through a funnel device or by hand, depending on the implant type and size.
  4. The surgeon closes the incision with sutures, tape, or glue, and covers the wound with a dressing or bandage.
  5. The surgeon may also place a surgical bra or compression garment over the breasts, to support and protect them during the healing process.

1) Implant Type

Saline versus Silicone Implants

They are designed to create a more natural and subtle result, especially in patients with little or no breast tissue, or in patients who want a low or moderate profile implant. They may also create less upper pole fullness, which may be preferred by some patients. However, anatomical implants may also be more expensive, more difficult to insert, and more prone to rotation or malposition, which can distort the breast shape and require revision surgery. They may also create less cleavage and projection, which may be undesirable by some patients.

In Canada, we used to offer a choice between teardrop (anatomic) or round implants for cosmetic breast augmentation. In recent years, we have discovered that the textured surface of teardrop implants may be associated with rare but serious complications. Accordingly, Dr. Pirani no longer offers teardrop implants to his patients. Your safety always has been and always will be Dr. Pirani’s foremost priority. Accordingly, Dr. Pirani currently only offers smooth round implants for cosmetic breast augmentation. Fortunately, Dr. Pirani is still able to achieve that natural teardrop appearance in most patients by placing the implants under the muscle. In doing this, the muscle pushes against the upper pole of the breast implant and helps create a teardrop appearance.

2) Breast Implant Size

Choosing a breast implant size is one of the most important decisions for breast augmentation patients. The size of the implant can affect the appearance, comfort, and satisfaction of the patient. There are several factors to consider when choosing a breast implant size, such as:

  1. The patient’s body type: The implant size should be proportionate to the patient’s height, weight, chest width, and shoulder width. A patient with a small frame may not be able to accommodate a large implant without risking complications or unnatural results. A patient with a large frame may need a larger implant to achieve the desired fullness and projection.
  2. The patient’s lifestyle: The implant size should be compatible with the patient’s physical activity, clothing preferences, and personal goals. A patient who is very active may prefer a smaller implant that does not interfere with exercise or sports. A patient who wants to wear revealing or fitted clothes may prefer a larger implant that enhances their curves and cleavage. A patient who wants a subtle or dramatic change may choose a different implant size accordingly.
  3. The patient’s expectations: The implant size should match the patient’s vision of their ideal breast size and shape. A patient should have realistic expectations of what breast augmentation can and cannot do, and should communicate their wishes clearly to their surgeon. A patient can use various methods to visualize their potential results, such as trying on different bra sizes, using sizers or implants during consultation, or using computer imaging or simulation.

It is important to remember that it is not possible to guarantee a specific post-operative size in terms of bra sizes. Bra sizing varies greatly between different bra manufacturers. A size D in one brand can be a full C in another brand for example. Dr. Pirani encourages you to bring a close friend or partner with you to your sizing appointment if you would like their opinion.

3) Breast Implant Incision Placement

Breast implant incisions are the locations where the surgeon makes the cuts to insert the implants. There are four main types of breast implant incisions: inframammary, periareolar, transaxillary, and transumbilical. Each type has pros and cons, such as:

  1. Inframammary incision: This incision is made near the crease under the breast, where the breast meets the chest wall. This is the most popular and versatile type of incision, as it allows the surgeon to have direct access and visibility to the breast pocket, and to insert almost any size and shape of implant. It also has less risk of infection, nipple sensation changes, or breastfeeding difficulties. However, this incision may also result in a visible scar under the breast, especially in patients with small breasts.
  2. Periareolar incision: This incision is made around the lower edge of the areola, the dark area around the nipple. This type of incision may result in a less visible scar, as it blends with the color and texture of the areola. It may also allow the surgeon to adjust the position or size of the areola, if desired by the patient. However, this incision may also have a higher risk of infection, capsular contractures, nipple sensation changes, or breastfeeding difficulties, as it involves cutting through the breast tissue and ducts. It may also limit the size and shape of the implant, as it requires a smaller opening.
  3. Transaxillary incision: This incision is made in the armpit, where the arm meets the chest. This type of incision may result in no visible scar on the breast, as it is hidden in the natural fold of the armpit. It may also avoid any damage to the breast tissue or ducts, and preserve the nipple sensation and breastfeeding function. However, this incision may also have a higher risk of implant malposition, asymmetry, or rotation, as it involves a longer and less direct route to the breast pocket. It may also limit the size and shape of the implant, as it requires a smaller opening.
  4. Transumbilical incision: This incision is made in the navel, or the belly button. This type of incision may result in no visible scar on the breast, as it is hidden in the navel. It may also avoid any damage to the breast tissue or ducts, and preserve the nipple sensation and breastfeeding function. However, this incision may also have a higher risk of implant malposition, asymmetry, or rotation, as it involves a longer and less direct route to the breast pocket. It may also limit the size and shape of the implant, as it requires a smaller opening and can only accommodate saline implants. It may not be widely available or accepted by many surgeons. Dr. Pirani does not offer this option.

The best breast implant incision for each patient depends on several factors, such as the implant type and size, the patient’s anatomy, the quality of the skin and tissue, the patient’s preferences, and the surgeon’s expertise. In general, the inframammary fold incision offers the surgeon the greatest ability to visualize and precisely develop a pocket for your specific implant. 4) Breast Implant Placement (Plane of Insertion)Breast Implant Over or Under

Breast implant placement refers to the position of the implant in relation to the chest muscle and the breast tissue. There are four main options for breast implant placement:

  • Subglandular placement: The implant is placed under the breast tissue but above the chest muscle. This option may be suitable for patients who have enough natural breast tissue to cover the implant, who want a more prominent and rounded shape, and who have less physical activity. However, this option may also result in more visible implant edges, more interference with mammograms, and a higher risk of capsular contracture.
  • Submuscular or subpectoral placement: The implant is placed under both the breast tissue and the chest muscle. This option may be suitable for patients who have thin or sagging skin, and who want a more natural shape.

Generally, subpectoral augmentation is associated with:

    • A lower incidence of capsular contracture (thick scar formation around the implant)
    • Decreased rippling
    • Maximal preservation of nipple sensation
    • Less implant visibility and palpability
    • Less tissue stretch over time
    • Better mammogram visualization

However, this option may result in more movement of the implant during muscle contraction (animation).

  • Subfascial placement: The implant is placed below the breast tissue and above the pectoralis major muscle, but under the thin layer of tissue called the fascia that covers the muscle. This technique can provide some advantages over subglandular or submuscular placement, such as limiting implant distortion or animation, providing extra soft tissue coverage and support, and reducing postoperative pain and recovery time. However, this technique may not be suitable for patients with thin skin or little breast tissue, as it may not provide enough coverage or concealment for the implant. Subfascial placement may also not be possible if your fascia is too thin to provide any benefit over subglandular placement.
  • Dual-plane placement (a type of submuscular or subpectoral placement): The implant is partially placed under the chest muscle and partially under the breast tissue. This option may be suitable for patients who want to combine the benefits of both subglandular and submuscular placement, such as a natural shape, improved implant coverage, and less complications. However, this option may also require more surgical skill and experience, and may not be available for all implant types and sizes.

The best breast implant placement for each patient depends on several factors, such as the implant size and shape, the patient’s anatomy, the quality of the skin and tissue, and the patient’s expectations. Dr. Pirani regularly performs dual plane subpectoral breast augmentations in addition to offering complete subglandular and subfascial placements, altering the technique he uses depending on your unique anatomy and aesthetic goals.

Dual Plane Breast Augmentation

The “No Touch” Technique of Breast Implant Insertion

The “no touch” technique of breast implant insertion is a method of placing the implant into the breast pocket without touching it with the surgeon’s hands or instruments. The implant is delivered through a funnel device (such as the Keller funnel), and is pushed into the pocket with gentle pressure. This technique can reduce the risk of infection, capsular contracture, and implant contamination.

Some of the benefits of the “no touch” technique are:

  1. It can minimize the contact between the implant and the skin, which can lower the chance of bacterial transmission and infection.
  2. It can prevent the implant from being damaged or scratched by the surgical tools, which can cause implant rupture or leakage.
  3. It can reduce the trauma and inflammation to the breast tissue and the implant, which can lead to capsular contracture, a condition where the scar tissue around the implant tightens and hardens.
  4. It can shorten the surgery time and the recovery time, as it involves less manipulation and bleeding.

Some of the drawbacks of the “no touch” technique are:

  1. It may not be compatible with all types and sizes of implants, especially large or textured implants, as they may not fit through the funnel device.
  2. It may not be suitable for all types of incisions, especially periareolar or transumbilical incisions, as they may not allow enough space or access for the funnel device.

The “no touch” technique of breast implant insertion is a safe and effective option for breast augmentation, and is used by Dr. Pirani for most cases.

Breast Augmentation Recovery

Breast augmentation recovery is the period of healing and adjustment after the surgery. It typically takes six weeks, but may vary depending on the patient’s health and healing rate. The recovery process may involve some pain, swelling, bruising, and tightness in the chest, which can be managed with pain medication, and conservative measures. In general, most patients will remain in the recovery area for a few hours before being allowed to go home the same day. You must have an adult accompany you home from our facility and stay with you for at least 24 hours after surgery. We generally recommend that you have someone at home to help you for at least the first week after surgery.The recovery process typically also involves some restrictions and precautions, such as:

  • Wearing a surgical bra or compression garment for several weeks to support and protect the breasts, and to reduce the swelling and bleeding.
  • Avoiding strenuous activities, heavy lifting, and exercise for at least four to six weeks, to prevent any pressure or trauma to the breasts, and to allow the implants to settle and stabilize.
  • Sleeping on the back to reduce the swelling and discomfort, and to avoid putting any weight on the breasts.
  • Keeping the incisions clean and dry, and applying antibiotic ointment or cream, to prevent infection and promote healing.

Most patients are able to return to work a week after receiving a breast augmentation. Patients should expect to continue to see changes in their breast shape and implant position for at least six months after surgery.

Breast Augmentation Risks

Although breast augmentation is a safe and common procedure, it is not without risks, and patients should be aware of them before undergoing breast augmentation surgery. Some (but not all) important risks are listed below:

  • Infection: This is a rare but serious complication that can occur after surgery. Should an infection occur, additional treatment including antibiotics, hospitalization, or additional surgery may be necessary. It is possible to lose your implants due to an infection.
  • Bleeding: It is possible, though unusual, to experience a major bleeding episode during or after surgery. Should post-operative bleeding occur, it may require emergency treatment to drain accumulated blood. Increased activity too soon after surgery can lead to an increased chance of bleeding and additional surgery. The risk of bleeding can be reduced by avoiding aspirin, ibuprofen, or other anti-inflammatory drugs, and by following your surgeon’s instructions regarding activity and medications.
  • Capsular contracture: This is a complication that can occur if the tissue that forms around the implant tightens and squeezes the implant. It can cause pain, hardness, distortion, or displacement of the implant, and may require surgery to release or remove the scar tissue or the implant. No. In Canada, we do not currently recommend implants be replaced unless there is a problem with them.

Do Breast Implants Cause Breast Cancer?

There is an association between textured implants and a rare form of lymphoma (ALCL). Dr. Pirani does not offer textured implants and has never used them since being in independent practice.

Breast Implant-associated Squamous Cell Carcinoma is a rare type of cancer associated with breast implants. It is important to investigate any changes in your breasts after breast augmentation with implants.

What Is Capsular Contracture?

When you place a foreign object in the body, such as a breast implant, the body reacts by creating a layer of tissue around the foreign object. We refer to this as the capsule. In most people, it resembles a layer of wet tissue paper around your implant and is not an issue. In some rare cases, however, the capsule can thicken and begin to distort the implant and even become painful. We refer to this as a capsular contracture. This usually requires surgical correction. It can be a recurring problem in very rare cases. There are off-label options to help prevent the progression of an early capsular contracture such as using Singulair for up to 3 months. If you feel that your breasts are beginning to feel firm after a breast augmentation with implants, it is important to see Dr. Pirani immediately so he can initiate such preventative care.

How Long Will I Stay In The Surgical Facility Before I Can Go Home?

In general, patients will stay with us for a few hours before going home. Patients have the option of staying overnight in one of our private overnight stay rooms, which are luxuriously furnished. 2 nurses will care for you overnight if you do choose to stay in one of these rooms. There is an additional fee if you choose to stay overnight in our facility. If you prefer to stay in a local hotel, our office has obtained special rates at a number of local hotels so please ask our staff about these.

Can I Exercise After My Procedure? 

Very low impact exercise such as slow walking is highly encouraged for the first 6 weeks after your surgery. Generally, you should avoid anything high impact to avoid complications during this 6 week period.

Right after surgery: You actually should start walking around right after your surgery. You should not do anything extreme or strenuous, but taking frequent, brief walks will keep your circulation up and promote healing.

Six weeks later: You should be well enough to return to all of your normal exercise routines.

With saline implants, it’s easier to determine when a rupture has occurred because the implant will physically decrease in size. Silicone implants, however, usually do not visibly shrink when they have ruptured.

What Kind Of Anaesthesia Is Required For Surgery?

Dr. Pirani only offers procedures with general anaesthesia provided by a board certified anaesthesiologist. Our facility is fully accredited by the Canadian Association for the Accreditation of Ambulatory Surgical Facilities (CAAASF). Our surgical facility has also passed inspection by the College of Physicians and Surgeons of Ontario (CPSO). Our anaesthesiologists are staff anaesthesiologists at the Toronto East General Hospital.

How Long Does The Operation Take?

On average, the procedure will take 1 to 1.5 hours to perform. Some more complex cases may take more time. If a mastopexy or breast lift is also performed at the same time, then the surgery can take up to 4 hours.

What Sort Of Things Are Required For Post-Operative Care?

Breast Augmentation Bra

After your surgery, Dr. Pirani will provide you with specific post-operative instructions. You will also receive a handout detailing your post-operative instructions. Generally, the following can be expected following surgery:

• Bra worn 24-7 for 1 week
• Expect some discomfort for 2-5 days
• No underwire bras for 6 weeks
• No heavy lifting or strenuous upper body exercises for 6 weeks
• Your first follow-up appointment with Dr. Pirani will be 5-7 days after your surgery.

Do Any Stitches Need To Be Removed?

Dr. Pirani usually uses absorbable sutures that do not need to be removed. In the rare case where a patient is prone to poor scarring, he will use non-absorbable sutures that are usually removed 7 days after the surgery.

How Do I Choose the Right Bra? 

Following your breast enhancement, you will need to purchase new bras. As your breasts will continue to change for several months after the surgery, we advise you to wait at least six to 12 weeks before purchasing a large number of new bras.

After surgery, Dr. Pirani will provide you with a post-operative surgical bra. During your follow-up appointments, he will reassess your bra requirements and direct you about what type of bra you can wear. In most cases, Dr. Pirani will have you continue to wear a supportive bra for the first six weeks after surgery; however, in some cases he may direct you to avoid wearing a bra to help your implants descend. In some cases, Dr. Pirani may request that you wear an underwire bra.

When it is time to purchase a new set of bras, it is very important to purchase bras that fit well. There are several specialized stores in Toronto that cater to women who have had a breast augmentation. Dr. Pirani and his team are more than happy to help you find the right bra following your procedure.

What implant size should I choose?

This is always a difficult decision for some. In my opinion, the best thing to do is have a thorough sizing session. I provide my patients with a range of about 3-4 implants that are appropriate for their specific measurements. I then have them try these implants on in a special sizing bra. With this process, my patients have been able to choose their own implants. I have found this to be the best method for implant sizing. Your surgeon should be willing to spend the time with you doing this.

What is a silent breast implant rupture?

This refers to an implant rupture or failure that is not evident clinically. That is, you may have an implant that has ruptured but not actually be able to tell from looking at or feeling your breasts. This is more common with gummy bear silicone breast implants as the implants are cohesive. These types of ruptures are usually found incidentally during breast ultrasounds and mammograms. Dr. Pirani generally recommends replacing implants when a silent rupture is detected.

Are There Any Increased Risks Associated With Larger Breast Implants?

Yes—larger implants generally lead to more sagging (ptosis) and this occurs earlier than with smaller implants. They may also lead to increased thinning of your natural breast tissues over time and are more likely to bottom-out over time. Dr. Pirani finds that implants over 400cc generally lead to increased complications over time but this does vary by body type and tissue quality.

Can I Be Awake For My Breast Augmentation Surgery?

There are surgeons that will do awake surgery for breast augmentations, but I do not. Most surgeons in North America do not offer awake breast augmentation surgery. There may be a cost advantage to being awake; however, I believe that the safety and comfort of my patients is paramount. Accordingly, I prefer to have my patients asleep for breast augmentation surgery.

What Exactly Does The Phrase “The Pocket” Refer To In Breast Augmentation?

The pocket refers to the space that is created for the implant to be inserted into. We have to create a space in which to place the implant—that’s the pocket.

How Do I Sleep After My Breast Augmentation?

Sleeping on your back is your best option in the early post-operative period. Sleeping on your side is your next best option. I tell my patients that sleep will be an issue in the first few weeks after breast augmentation surgery, as you now have two weights on your chest which can make it more difficult to fall asleep.

Will My Nipple Sensation Be Altered By Having A Breast Augmentation?

The literature would indicate that there is a 15% chance of losing sensation to your nipple-areolar complex. Having said that, I think the numbers in the literature are very high. I certainly haven’t had this high a rate in my patients.

Although the rates of sensation loss may be as high as 15%, a significant proportion of these cases will be temporary. It all depends on the degree of injury to the nerves that provide sensation to your nipple. If the nerves are just bruised (neuropraxia), your sensation should return within 6-12 weeks. Increased sensation or hypersensitivity is also a possibility.

How Much Pain Will I Have After a Breast Augmentation with Implants?

Recovery really depends on a few factors. Adequate pain control is likely the most important. Individual pain tolerances differ greatly. I find that women who have had children generally have less post-op pain, as they have a much higher pain tolerance. From a surgical point of view, there are several things the surgeon/anaesthesiologist can do to decrease your post-op pain.

I generally perform breast blocks prior to making any incisions with a mix of short- and long-acting local anaesthetics. I have found this greatly reduces post-op pain. I also encourage patients to take the prescribed pain killers regularly for the first 24-48 hours. If you don’t stay on top of the pain in the immediate post-operative period, it is extremely difficult to “catch-up”. Most of my patients experience very little post-op pain after their breast augmentation surgery.

Of course, the type of breast augmentation you have will also determine your post-operative discomfort. Larger implants, and subpectoral implant placement will also increase post-op discomfort. This doesn’t mean you shouldn’t get large implants, or place them under the muscle—you simply need to know what to expect. Many women also experience difficulty with sleeping in the first few weeks after augmentation due to the weight of the implants on their chest.

Should I Avoid Breast Implants If I’m An Athlete Or Involved In Contact Sports?

Being an athlete or playing contact sports is not a contraindication for breast augmentation. Generally, contact sports may be resumed once you have healed well following breast augmentation. You may generally resume exercise and routine sports after 6 weeks following breast implant surgery.

Am I Too Skinny Or Athletic For Breast Implants?

Slender patients with limited breast tissue can still be great candidates for breast augmentation with implants. Having less soft tissue coverage does increase the risks of implant visibility, rippling, and implant palpability. Nonetheless, these risks can be mitigated by placing the implants under the pectoralis major muscle and not choosing extra high projection implants. Adding fat grafting when possible to augment tissue coverage can also help with achieving natural appearing breast augmentation results. It is important to discuss your aesthetic goals with Dr. Pirani so he can help you get the best aesthetic results.

Can Breast Implants Rupture And Leak Silicone In My Body?

As with any mechanical device, breast implants will eventually fail and/or rupture. Fortunately, the newest generation of silicone implants have very low rupture rates and excellent longevity. In addition, the new cohesive gel silicone (gummy bear) implants make it much less likely that silicone will leak in the event of a rupture.

Can I Move My Arms After Breast Augmentation?

Dr. Pirani generally prefers that you keep your elbows at the sides of your body for most activities during the first 6 weeks after your surgery. You will have to alter how you perform regular activities during this period but most patients are able to cope quite well once they get used to this restriction. Washing your hair is the most common issue due to this restriction so we recommend getting help from a friend or family member. Dry shampoo products are also quite helpful.

Are Breast Implants Under Warranty?

Breast implants are generally covered by a manufacturers warranty for their lifetime. Following breast augmentation surgery, you will receive an implant registration card that lists your implant size, model, type, and serial number. It is important that you save this in a secure location for future claims. Patients are encouraged to take the time to review the details of their specific implant warranties.

Do Breasts Sag After Breast Augmentation?

Breasts certainly may sag after breast augmentation and this may be related to the increased weight added to the breasts by the implants and/or normal tissue aging, pregnancy, breastfeeding, weight changes, UV exposure, and nicotine use. Larger and heavier implants are more likely to develop sagging or ptosis with time. There is no way to predict when this will happen. 

After breast augmentation surgery with implants, it is recommended that you wear a supportive bra as much as possible to limit ptosis. This is especially important when performing high-impact activities.

How Do I Know If My Breast Implant Is Leaking or Has Ruptured?

This will depend on the type of breast implant you have. Saline implant ruptures are usually associated with a sudden loss of volume although it is possible that a small saline leak could result in a gradual decrease in size over a few days. Cohesive gel silicone implants are more likely to have silent ruptures if a small defect is present. Silicone implant ruptures are best detected through imaging modalities such as ultrasound or MRI scans.

What Is A Normal Breast Shape?

Although breasts come in many different shapes and sizes, a “natural” breast classically has a teardrop shape with a sloped upper breast and a rounded or fuller lower breast. Having said this, many women actually prefer a more rounded upper breast and/or a round appearance throughout the breast. Dr. Pirani will take the time to understand your aesthetic goals and help you achieve these goals.

Can Breast Implants Last 30 Years?

Breast implants can potentially last decades. Having said that, we do not know how may decades the new generation of implants will last as they haven’t been around for 30 years.

Can Breast Implants Cause Pain Years Later?

Capsular contractures, implant ruptures, and breast implant illness are but a few potential causes for breast pain years after breast augmentation with implants. If you have late-onset breast pain you should always first see your primary care physician to investigate and determine the cause. If indicated, they can refer you to your surgeon if the implants are the source of your pain.

Does Getting Breast Implants Make You Gain Weight?

Most weight gain from breast implants is usually limited to the weight of the implants themselves. This is usually minimal unless very large implants are chosen. It is important to maintain healthy lifestyle choices after surgery as you will be not be allowed to perform high-impact exercises for 6 weeks after surgery.

Does Insurance Cover Breast Implants?

Breast implant surgery is usually considered cosmetic surgery and therefore not usually covered by most insurance providers. Dr. Pirani does not perform OHIP procedures and does not accept payment directly from any insurance providers for cosmetic procedures.
